Who we are:

The New Life Counseling website is a portal to serve patients and clients for office visits within the vicinity of  Midwestern Oklahoma, generally within a 75 mile radius of Clinton and Cordell, OK. Teleconference is available throughout the state of Oklahoma.  The website address is: https://new-life-counseling-oklahoma.com.

Comments

In addition to providing information about services provided by New Life Counseling, a consumer may find blog articles, YouTube videos, special presentations and links to outside sources of information.  Some may be set up to enable comments.  Participants are encouraged to use discretion in their remarks and to respect their own and others privacy and confidentiality.  Any client or former client, who, on his or her own initiative would like to write a positive testimonial about services received from NLC may wish to do so without giving a full name and without mentioning the name of anyone else. It is important to think through ramifications before leaving an online comment.  Thank you advance for any positive mentions.

A counseling relationship is initiated (but not confirmed) by scheduling a free conference call and/or scheduling an Intake Session. Another step is taken by filling out the INTAKE FORMS which should be completed before the start of the first session. The Intake Form is considered highly confidential and should be completed, scanned and emailed to rsw@new-life-counseling-oklahoma.com (HIPAA complaint interface with Google Suite) or faxed through 888-344-9087 (hosted through a secure service). 

Media

An earnest attempt has been made to identify and utilize linked services for cost-effective, private scheduling, provision of client information to be utilized in coaching to include teleconferencing and texting. More about these vehicles can be found in the AGREEMENT.

Forms

The INTAKE forms contain questions to help your practitioner provide optimal service and information about this practice. They may be downloaded, examined and completed and then faxed to 888-344-9087, delivered In-Person, or mailed to address that can be provided.  The fax number is hosted by SRFax which offers encryption and other HIPAA safeguards to ensure protection of privacy.  A faxed item may remain on the SRFax service during the time it is retained which may be several years.  Forms are printed out on a HP Printer that is sustained by the HP Instant Ink Service.

Cookies

The owner of this site is aware of cookie use from WordPress Plugins and is under the impression that nothing other than superficial visitor profile is obtained from Google Analytics, JetPack and other entities such as Facebook.  At least some plugins and various interfaces may use cookies in a similar manner as Google Analytics.  Most websites including this one often change plug-ins and third party interfaces.  Such changes may or may not be reflected in any given instance of a privacy policy. Embedded content from other websites

As of 7.9.19 the owner of this site has manually embedded html codes (or receptors for codes) that interface a WordPress shell with the following services: select components of HIPAA approved Google Business Suite, Square, Pcloud, Doxy.me and Elementor and accompanying theme.  At the time of this update, 8.7.19, a theme from Phlox was in use. 

Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.  It is possible that a newly installed application or one not identified in this policy is somehow embedded or in some way attached to this site and able to obtain information on visitors and attach cookies to track online behavior.

Most substantial websites are constructed or interfaced in some manner attended by applications that may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracking your interaction with the embedded content if you have an account and are logged in to that website.

Who we share your data with

No data voluntarily provided by clients on intake forms or other self directed conveyance are voluntarily made available to any third party without the express written consent of patient or client.  See AGREEMENT for extraordinarily rare exceptions such as a court order or separate agreements involving groups or collectivities voluntarily consenting to participate in a service promotion.

How long we retain your data

Data voluntarily provided by client’s in alignment with counseling services may be kept indefinitely in a secure location and should be disposed of and/or expunged in the event of practitioner’s death or sustained disengagement from practice. 

What rights you have over your data

Any legally competent patient or client who wishes to discuss a pathway for the removal of information from client file may contact owner of site or designee.

Where we send your data

Unless client provides written authorization to send data to a third party, there is no design willfully orchestrated by this practitioner to send data to any source outside this practice. Obviously there are tools that collect superficial visitor information that are described in other sections of this Privacy Disclosure Notification

Additional information

Physical location is private property owned by practitioner in a small, law abiding community in western Oklahoma.  It retains hard copy information in designated area(s).  This practice functions with electronic devices that process digital information such as emails and interfaces with earlier mentioned online services and possibly a service not identified.  At the time of 8.7.19 update, the phone service is provided by T-Mobile and Internet Service by Sparklight – a rebrand of Cable ONE.

How we protect your data

Physical sources, including those that are electronic devices that contain or convey PHI are not fully identified as a means to help protect PHI. They are known to OCHA [Oklahoma Health Care Authority]. In order to protect security. additional information pertaining to venues of operation and means of protection are not provided in this public policy disclosure.  

What data breach procedures we have in place

This website and its components are routinely examined, upgraded and updated.  The administrator seeks to evaluate and compensate for risks to PHI and facilitate other consumer protections in everyday operations.  If potential for a data breach is made known to this administrator, then related Internet Service Provider(s) will be notified and a good faith attempt to correct and compensate will be made. 

What third parties we receive data from

See embedded content section.  Social media channels include Facebook, YouTube, LinkedIn and Twitter.  This business may use such services to draw patients/clients to consider the services of New Life Counseling.

What automated decision making and/or profiling we do with user data

As of 5.14.19, the primary decision making model is to respon to direct inquiries from visitors to website and advertising channels and to orchestrate exposure of said services through social media and/or advertising designed to identify and engage populations determined to have need(s) for the service(s) in the catchment area of New Life Counseling.

Disclosures in this Privacy Policy are provided in good faith to comply with regulations associated with HIPAA, OHCA and other entities that regulate commerce over the internet.
